Pothole on pavement on even number side of The Avenue

We were walking back from Orpington Station at about 2300 hours on Sunday night and my husband stepped on the grass verge that sticks out and went flying across the pavement. This is a very deep hole and due to the fallen leaves was hard to notice, not to mention that the lighting is bad due to the branches of the trees. Luckily he wasn't hurt but has a bruise on his elbow where he landed, given he had a thick coat, jumper and long sleeved woollen shirt as it was very cold. Please fill in this hole as we often come back late and we don't want another accident to happen. Please also try and cut back the branches near the trees and clear the fallen leaves as these get slippery in the rain. Also the narrow pavement leading from Tower road to the station is very slippery with fallen leaves and I fear there could be an accident and the road should be swept as a priority frequently as it is very busy. This area is across the road from nos 19/21 the avenue and outside a fenced back garden.
